EPUB files only. TOC is taken directly from the EPUB (NCX / nav).
Adds chapters found in spine files the NCX/nav TOC omitted (e.g. Calibre-generated EPUBs where each sub-chapter is its own file). Detected headings shown in italics.
Forces use of the EPUB3 nav (toc.xhtml) even when the manifest doesn't mark it with properties="nav". Fixes duplicate chapters in some EPUBs with malformed NCX files.
Removes consecutive TOC entries pointing to the same file and fragment. Use when chapters appear twice due to a malformed NCX or nav (e.g. pirated EPUBs).
Splits large TOC entries at typographic section breaks for per-section summarization. Affects summarization only; EPUB splitting still operates at file boundaries. Mutually exclusive with Deep scan.
